The quickest way to begin the admissions process is to download a PDF version of the LAFS application. However, you must have Adobe Acrobat Reader® to view and print the files.

If you are unable to access the document for any reason, please call the Admissions Department at 877.952.3456 to request an enrollment application. Once you receive the enrollment application, simply complete it and return it to your Admissions Representative.
